I would be happy to answer any of your questions and set a time for our initial appointment. I am a provider for most of the major medical insurance companies.
If you want to fill out your paperwork for me before you come to the office, click on the site Initial Assessment Forms. You can download them and bring them with you on your first visit. If you would rather wait to fill the forms out in the office, come about 10 minutes early and I’ll have the forms out in the waiting room.
